MEDICAL FACILITIES IN STATE BANK OF PAKISTAN,

SBP BSC (BANK) AND NIBAF

            State Bank provides medical facilities to its regular, part-time as well as retired employees and their dependent family members as also to the dependents of the deceased employees. The dependents include wife, legitimate children, step children, adopted children (in case of non-muslim employees only) and parents residing with and wholly dependent upon him / her.

2.                  Medical facilities include the following:-

i)                    Examination by the Bank’s doctors at dispensaries situated at the centre of his / her posting and provision of medicines prescribed by them from the dispensaries.

ii)                  In case of non-availability of any prescribed medicine/s at the dispensary, provision of that / those from the approved Medical Stores.

iii)                 Consultation with specialists borne on the approved panel of the Bank if considered necessary by the Bank’s Medical Officer / Chief Medical Officer / Officer-in-Charge of an office or branch of the Bank.

iv)                Pathological / Radiological and other methods of examination from the approved Pathologists / Radiologists etc. if considered necessary by any of the above officers.

v)                  Hospitalization in approved hospitals / clinics / maternity homes if considered necessary / permitted by any of the above officers. In case of emergency the condition of prior permission shall not apply. However, admission will have to be got regularized from any of the above officers within 48 hours.

vi)                Unani, Ayurvedic or Homoeopathic treatment from practitioners approved by the Bank subject to reimbursement of cost of such treatment as per Bank’s rules / approved rates.

vii)               Consultation with specialists and Unani / Ayurvedic / Homoeopathic practitioners, (ii) examination by   Pathologists / Radiologists etc and (iii) hospitalization in hospitals / clinics / maternity homes not approved by the Bank with prior permission of any of the above officers subject to reimbursement of cost thereof at par with Bank’s approved rates. In case of hospitalization in emergency the condition of prior permission shall not apply. However, admission will have to be got regularized from any of the above officers within 48 hours.

viii)             Outstation medical facilities at the request of the employee.

ix)                Emergency medical treatment from a private doctor other than specialist for 24 to 48 hours subject to reimbursement of cost thereof as per Bank’s rules / approved rates.

3.         3. Governors / Deputy Governors (in-service as well as retired) and their dependent family members are entitled to actual (full) medical facilities which may include hospitalization in un-approved hospitals, consultation with specialists and purchase of medicines from Medical Stores backed by prescriptions of specialists and outside Medical Practitioners without reference to Bank’s Medical Officer / Chief Medical Officer.

4.       4.      Medical facilities to the dependent sons are admissible upto the age of 28 years. They cease to be dependents for medical facilities when they get married or get a gainful employment or attain the age of 28 years whichever is earlier.

5.                                 5.Un-employed / unmarried / divorced daughters of an employee are entitled to medical facilities without age limit.

6.         6.                     The dependent parents of an employee are eligible for medical facilities subject to the condition that his / her father has attained the age of 60 years and his income does not exceed Rs.10,000/- per month. However, the condition of age will not apply in case of a widowed mother. The dependent parents must have been residing permanently with the employee. The employee will have to furnish a Declaration to this effect. In the case of handicapped / disabled father of any age of an employee the matter will be referred to a Medical Board headed by Director, Medical Services Department and decision will be made in the light of its recommendations with the approval of Managing Director / Deputy Governor.

7.         The un-employed / handicapped husband of a female employee is entitled to medical facilities with the approval of Managing Director / Deputy Governor.

8.            Medical facilities to dependent parents of female employees are continued even after their marriage.

9.         Routine emergency laboratory tests such as Blood CP ESR, Urine DR, Blood Sugar etc. advised by consultants / local doctors can be carried out without the approval of Medical Services Department / concerned office subject to reimbursement upto a maximum of Rs. 1,000/-. Reimbursement will, however, be made at par with the Bank’s approved rates.

10.       Employees who get treatment / admission in un-approved hospitals such as Aga Khan, O&MI, Mid East etc. at their own expense, are entitled to claim reimbursement to the extent of 70% of the expenditure incurred. However, prior permission of Medical Services Department will be required in all such cases except in case of emergencies where regularization of admission will be obtained within 48 hours of admission.

11.       In cases of emergency like acute myocardial infarction, fatal road accident, gun  shot wound and severe burns etc., cost of full treatment obtained at any hospital according to entitled accommodation is allowed provided regularization of admission from Medical Services Department / concerned office is obtained within 72 hours of admission.

12.              Hospitalization to Bank’s employees and their eligible dependents is allowed as follows:-

i)                    Officers in OG-1 and above on the basis of categories / grades as under:-

13.       Employees appointed on contract basis and those appointed on daily wages basis are also entitled to medical facilities from the Bank.  Contract employees are allowed medical facilities as specifically mentioned in the terms and conditions of their appointment while daily wages employees are allowed medical facilities for self only to the extent as available in the Bank’s dispensary.

14.       In Karachi and some other centres, female dispensaries under the supervision of lady doctors are also working for the treatment of the ladies and children. This is an additional facility provided by the Bank.    

15.       For availing of medical facilities from the Bank, an employee has to fill form ED-77  i.e. “Declaration of family members for the purpose of medical facilities” in triplicate, get it countersigned from the concerned Head of the Department / Chief Manager and hand over it to the concerned official dealing with the medical facilities at a particular centre alongwith copies of his appointment letter and terms and conditions of his appointment etc. The above form is available at the Administration or Staff Sections of the Offices. In Karachi it is also available at (i) Human Resources Department, (ii) Administration Department, SBP BSC (Bank), Head Office and (iii) Medical Services Department, SBP BSC (Bank), Head Office.

16.       Medical claims are required to be submitted by the employees on the prescribed form “A” or “A-I”. Form “A” is meant for “Allopathic Treatment” and form “A-I” for “Unani / Homoeopathic Treatment”. These forms are available in the Offices as well as in Medical Services Department from where they can be obtained.
